What Is Medicaid Service CoordinationA Service Coordinator provides assistance in navigating regulatory systems such as the New York State Education Department (SED), Office of People With Developmental Disabilities (OPWDD), New York State Department of Family Assistance and Medicaid to assure that the people they serve have access to comprehensive medical, clinical, educational, vocational and residential services as needed.  A Service Coordinator is an individual's and family's advocate, coordinating all team members in a  goal-orientated service plan which ensures the best services for the individual.

Our Service Coordinators are well versed with  skills and abilities in order to provide quality service. These include:

  • Understanding person centered planning
  • Listening carefully to help the person and his/her family
  • Recognizing and addressing health and safety issues
  • Excellent communication skills for better service
  • Helping resolve crisis situations
  • Helping access entitlements and benefits
  • Advocating for the family and individual needing help
  • Help to ensure quality of life
  • Maintaining paperwork to better service the consumer
  • Helping find needed services and supports

Community Habilitation


Community Habilitation is a new HCBS Waiver service designed to promote independence and community integration by offering skills training and other habilitative supports which take place in non-certified every day community settings.
